\n\t\t\t\t\tAttempts at coming up with a method to achieve successful dental implants have been around for a very long time. In fact, since the 1700s scientists and dentists have been hard at work trying to find a successful method of implanting a false tooth into a human jaw to replace a missing tooth. These include the earliest (unsuccessful) attempts of taking a dead person\u2019s tooth and sticking it in the extraction socket of a living person; to years of trial and error using different tooth root shaped designs of different metals and ceramics and finally to today\u2019s highly technical modern dental implants with 92%-99% success rates.\n\nOne innovation directly related to these implant advances was discovered through research and named \u201cAll on 4\u201d. This technique is a real breakthrough for patients who wear full dentures. This unique means of utilizing implants eliminates the problems inherent in patients having to wear removable full or \u201ccomplete\u201d dentures. Some of these inherent problems include:\n\n

    "Tous sur 4"<\/h2>\n\n\u00ab\u00a0All on 4\u00a0\u00bb d\u00e9crit parfaitement la proc\u00e9dure. Il s'agit de fixer un ensemble complet (TOUTES) de dents artificielles non amovibles sur quatre (4) implants dentaires\u00a0; \u00ab\u00a0All on 4\u00a0\u00bb.\n\nproc\u00e9dure dentaire<\/strong>\n\nLa proc\u00e9dure est simple et les patients peuvent b\u00e9n\u00e9ficier d\u2019un ensemble de dents temporaires (provisoires) plac\u00e9 imm\u00e9diatement (24 \u00e0 48 heures) apr\u00e8s la chirurgie.\n\n
      \n
    1. Chaque patient b\u00e9n\u00e9ficiera d'un examen bucco-dentaire complet et d'une consultation.<\/li>\n
    2. Des radiographies sp\u00e9ciales sont prises pour \u00e9valuer la quantit\u00e9 et l\u2019\u00e9tat de l\u2019os en vue de la planification du traitement implantaire.<\/li>\n
    3. Un ensemble temporaire de dents de remplacement non amovibles, de forme et de fonction similaires \u00e0 la restauration d\u00e9finitive, sera fabriqu\u00e9 par un laboratoire dentaire avant la pose de l'implant. Si le patient poss\u00e8de une proth\u00e8se dentaire acceptable, celle-ci pourra servir de proth\u00e8se temporaire.<\/li>\n
    4. Un traitement antibiotique est g\u00e9n\u00e9ralement d\u00e9but\u00e9 2 jours avant l'intervention et poursuivi pendant dix jours apr\u00e8s.<\/li>\n
    5. La plupart des patients re\u00e7oivent une s\u00e9dation consciente par voie intraveineuse et\/ou du protoxyde d'azote et une anesth\u00e9sie locale pendant toute la dur\u00e9e de l'intervention. Certains patients peuvent n\u00e9cessiter une anesth\u00e9sie g\u00e9n\u00e9rale. <\/li>\n
    6. L'os est expos\u00e9 et inspect\u00e9 pour s'assurer qu'il convient aux implants et s'il y a des dents qui doivent \u00eatre retir\u00e9es, elles seront extraites.<\/li>\n
    7. Les quatre implants sont plac\u00e9s sur chaque arcade\u00a0: deux \u00e0 l'avant et deux \u00e0 l'arri\u00e8re. Les deux \u00e0 l'arri\u00e8re sont inclin\u00e9s, ce qui permet \u00e0 seulement quatre implants de supporter une dentition artificielle compl\u00e8te.<\/li>\n
    8. Le tissu gingival est sutur\u00e9 autour des implants.<\/li>\n
    9. La proth\u00e8se temporaire est ajust\u00e9e et fix\u00e9e sur les 4 implants.<\/li>\n
    10. Des visites de suivi sont pr\u00e9vues \u00e0 1 semaine, 2 semaines, 3 semaines et 3 mois.<\/li>\n
    11. Lors du rendez-vous de 3 mois, les implants seront inspect\u00e9s et si une gu\u00e9rison compl\u00e8te et correcte l'indique, la proth\u00e8se temporaire sera retir\u00e9e et le remplacement permanent sera fix\u00e9 sur les implants.<\/li>\n
    12. Des rendez-vous de suivi d\u2019entretien des implants sont programm\u00e9s. <\/li>\n<\/ol>\n\nDans certains cas rares, au moment de l'intervention chirurgicale, des conditions d\u00e9favorables et inattendues peuvent \u00eatre d\u00e9couvertes, n\u00e9cessitant la pose d'implants standards, retardant ainsi la pose d'une proth\u00e8se temporaire fix\u00e9e imm\u00e9diatement sur les implants.\n\n

      Implants standards<\/h2>\n\n\"\"\n\nStandard implants have many things in common with the implants used in the All on 4 procedure. The basic surgery is similar as are the shape, material, etc., of the implants.\n\nThe primary difference is that standard implants are placed into the jaw, and are covered back up completely by the gum tissue and allowed to heal for 3 months and the temporary replacement is not attached to the implants.\n\nDuring this healing time, a temporary removable denture will have been fabricated for patient use, or the patient\u2019s existing denture will be adjusted and refitted to serve as a temporary replacement. So, until the standard implants have healed enough for the attachment of the permanent set of teeth, the patient will be wearing a conventional type denture.\n\n

      Chargement imm\u00e9diat ou diff\u00e9r\u00e9<\/h2>\n\nThe All on 4 procedure uses a concept known as immediate loading. Immediate loading means that the patient receives a functioning temporary dental restoration that is placed at the same time as the implant surgery. In the case of the All on 4 full mouth procedure, the patient can eat, and function with a full set of temporary teeth which immediately places a force or load on the implants, hence immediate load.\n\nAdvantages of Immediate loading include:\n\n

      • Une seule intervention chirurgicale.<\/li>\n<\/ul>\n\nTorqueing\n\nDental implants need to be screwed into the channel made in the bone. The amount of pressure applied to the implant is called torque. As an example, if you have a flat tire and neglect to tighten the lug nuts enough the wheel may become loose and fall off. On the other hand, if you overly tighten the lug nuts, you may not be able to loosen them to take the wheel off.\n\nDental implants are similar in some ways to the lug nuts of a car. With implants, if you do not tighten the implant with enough torque, the implant has a greater chance of failure. Conversely, overtightening the implant also carries a risk of greater failure. To lessen this chance, dentists use a torque wrench or hand piece to precisely measure the correct torque which usually has a range of 30 ncm to 60 ncm. There are some disagreements among dentists concerning the range of torque.\n\nDelayed loading is defined as it sounds once the definition of immediate loading is understood. For various reasons, some implants cannot withstand any force being placed on them immediately until they have healed and are stable. In these instances, other temporary replacements are used that do not put any stress on the implants.
